One of the most common things women say to me in clinic is:
“I just don’t feel like myself anymore.”
They’re exhausted despite sleeping for eight hours.
They’re waking at 3am.
They’re more anxious than they’ve ever been.
They’re forgetting words, struggling to concentrate, putting on weight around their middle, and wondering why they suddenly can’t cope with life in the way they used to.
The answer they’re often given is simple:
“It’s just your hormones.”
Now, whilst hormones are absolutely changing during perimenopause, I believe that explanation only tells part of the story.
Perimenopause isn’t simply a hormonal transition.
It’s a whole-body transition.
And understanding that can completely change the way you approach your health.
Perimenopause affects far more than your ovaries
When we think about perimenopause, most of us think about declining oestrogen and progesterone.
Those hormones are certainly changing, but they don’t exist in isolation.
Oestrogen and progesterone interact with almost every major system in the body.
As they begin to fluctuate and decline, they influence your:
- Nervous system
- Stress response
- Sleep
- Blood sugar regulation
- Metabolism
- Brain function
- Mood
- Energy production
- Exercise tolerance
- Muscle health
That’s why symptoms such as anxiety, brain fog, fatigue, poor sleep and weight gain often appear together.
They’re not random.
They’re connected.
I call this The Whole-Body Shift.
Why you suddenly can’t cope like you used to
Perhaps one of the biggest worries women share with me is this:
“I used to cope with everything. What’s happened to me?”
I don’t think this means you’re becoming weaker.
I think your physiology is changing.
Imagine you’ve spent years carrying a backpack.
Over time you’ve gradually filled it with work, family responsibilities, poor sleep, stress, the mental load, rushing around, and putting everyone else’s needs before your own.
Your body has been remarkably good at compensating.
Then perimenopause arrives.
Without you even noticing, a few extra bricks are added to that backpack.
Suddenly, the same life feels much heavier.
The stressful week you once recovered from quickly now wipes you out.
One bad night’s sleep affects you for days.
Skipping lunch leaves you anxious and irritable.
A couple of glasses of wine impact you much more than they used to.
Nothing about you has become weaker.
Your body simply has less capacity to absorb stress than it once did.
Your body isn’t working against you
This is one of the biggest mindset shifts I hope women take away from my work.
Your body isn’t trying to make life difficult.
It’s communicating.
It’s asking for something different.
Perhaps it’s asking for more recovery.
More nourishment.
More strength.
Better sleep.
More time outdoors.
Less rushing.
Less constantly putting yourself last.
The strategies that helped you get through your twenties and thirties may no longer be the strategies that help you thrive in your forties and fifties.
That’s not failure.
It’s feedback.
Why focusing on hormones alone isn’t enough
Hormones are incredibly important.
For many women, HRT can be transformative and improve quality of life enormously.
But hormones don’t replace sleep.
They don’t replace good nutrition.
They don’t replace movement.
They don’t replace a well-supported nervous system.
That’s why I take a whole-body approach with my clients.
Alongside understanding hormones, we also explore:
- Sleep quality
- Blood sugar balance
- Stress resilience
- Movement and strength
- Nutrition
- Gut health
- Lifestyle habits
- Nutrient status
Because all of these influence how well your body adapts during perimenopause.
My Four Foundational Pillars
Almost everything I do with clients comes back to four simple foundations.
Blood sugar balance
Stable blood sugar supports energy, mood, concentration, hormone health and appetite regulation.
Sleep
Good sleep begins long before bedtime. Supporting your circadian rhythm and nervous system is one of the most powerful things you can do during perimenopause.
Stress resilience
Life will always contain stress.
The goal isn’t to eliminate it.
It’s to help your body become more resilient to it.
Movement
Not exercise as punishment.
Movement as nourishment.
Strength training, walking and regular movement support your muscles, metabolism, bones, brain and long-term health.
These four foundations work together to create a body that feels more resilient and better able to navigate the changes of midlife.
A different way of thinking about perimenopause
I believe we’ve spent too long viewing perimenopause as something to simply survive.
Instead, I see it as an invitation.
An invitation to begin listening to your body more closely.
To nourish it more consistently.
To build strength for the decades ahead.
To stop fighting your physiology and start working with it.
When we understand perimenopause as a whole-body transition rather than simply a hormonal one, everything begins to make more sense.
Instead of asking…“How do I fix this symptom?”
we can begin to ask…“How can I better support my body through this transition?”
For me, that’s a far more empowering question.
